Works under the direction of the Medical Director and Director of Operations for Churn Creek Health Care to perform a variety of office support including assisting the Churn Creek Health Care (CCHC) health care providers with standard documents and correspondence, relaying telephone inquiries, scheduling calendar items, meetings, processing forms and data entry. Position requires a working knowledge and ability to utilize the EMR/ EHR, EPM.

  •  Be dependable, reliable and possess organizational skills.
  •  1-3 years of clerical experience.

Recovery Case Manager

JOB SUMMARY:

The Recovery Case Manager will be part of the Recovery Program team and work directly with the Recovery patients and staff to effectively link patients to treatment options and other social services. The Case Manager will help patients navigating the healthcare system by providing case management, care coordination, outreach, support and linkage to care. The Case Manager may also provide health education services in the community to promote RRTHS services in Shasta and Trinity County, including assistance with the planning and operations of the syringe services program. The Case Manager will also be trained to assist as needed with other programs and services hosted by the recovery program. This is a versatile position that may require working in the field and with various departments within RRTHS.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

JOB SUMMARY:

The Behavioral Health Substance Abuse Counselor shall develop and maintain an outpatient treatment program and facilitate group meetings for individuals affected by substance use.
Treatment services should focus on engaging the client in health strategies to reduce persistent behavioral health and alcohol drug problems for Native American individuals, families, and their communities. The substance abuse treatment counselor shall provide clinical substance abuse counseling services through treatment modalities such as crisis intervention, individual and group counseling to educate, prevent or improve psychosocial problems, particularly pertaining to substance use.

  • CCAPP Certification

  • Assists patients in completing an Alternate Resource application including Medi-Cal and Partnership.
  • Check-in patients for Health Care Services, such as but not limited to, Family Practice, Urgent Care, Labs, and Registration.
  • Gather information from the patient to complete Alternate Resource filing and/or registration.
  • Update patient file with personal information or Alternate Resource information by appointment or walk-in.

JOB SUMMARY:

Responsible for developing, monitoring, implementing, and evaluating the tribes various energy and emergency assistance programs. Staff person will be responsible for prioritizing and registering energy and emergency assistance applications, scanning applications into electronic filing system and assisting with crisis energy program coordination as needed.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Associate’s degree or post-secondary training preferred.
  • One year of work experience in a position performing clerical functions in an office/service organization setting.
  •  Experience in human/social service organization preferred.

The Redding Rancheria is seeking a Behavioral Health Case Manager.

To provide supportive case management services for patients within the Behavioral Health Department of the Redding Rancheria Tribal Health Center.

 

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Must have a Bachelor’s degree in psychology or social work.
  • Documented experience working in a mental health related field.
  • Preferred experience in evaluating patient eligibility and Alternate Resources status.
  • Must be proficient in using Microsoft Office Suite programs including Word, Excel, Publisher, and PowerPoint to make spreadsheets, graphs, manage data and create brochures, presentations and documents on computers as needed.
  • Must pass background assessment of moral character and legal status. Must submit to fingerprint identification with law enforcement and be able to work with children.
  • Must demonstrate emotional stability, independent judgement, and initiative in professional activities and relationships with clients and co-workers.
  • Be in possession of valid and current California Driver’s License.

ESSENTIAL JOB FUNCTIONS:

  • Provides case management services, referral and coordination with other allied professionals and treatment providers, including Medical Providers, Psychiatrists, Therapists, Tribal Child and Family Services, School IEP Coordinators, Courts, Substance Abuse Treatment Facilities, and other community-based agencies.
  • Initiates and coordinates individualized care plan for clients, addressing both clinical and non-clinical components, and ensuring the care plan is available to the patient, primary care provider, Behavorial Health Therapist, psychiatrist, substance abuse counselor and/or other internal departments as needed on a case-by-case basis.
  • Provides ongoing support for clients while continually assessing their need for increased mental health services and providing crisis intervention when needed.
  • Advocates for the patient/family to ensure the most compassionate and proper care is given.
  • Gathers patient records and any other necessary documentation from external facilities for providers to use for treatment planning.
  • Coordinates treatment planning with the Recovery Department and Tribal Court as needed.
  • Ensures smooth implementation and continuum, of care via effective and frequent communication with clients or their identified health care designee(s), medical team members, and pharmacy technicians.
  • Coordinates referrals for services required to improve the client’s health status.
  • Maintains accurate and timely documentation, records, and case files in the NextGen system.
  • Manages and communicates lab results, referrals, and medication issues between providers and patients via clinical tasking.
  • Ensures follow up with patients regarding missed appointments and medication requests.
  • Rooms patients and records vital signs.
  • Reviews patient daily schedule for controlled medications and performs urine screenings for patients who are on controlled medications per DEA guidelines
  • Completes prior authorizations and appeals for patient medications.
  • Develops and maintains knowledge of a community-based network of alternative modes of care; aids clients to connect with community-based organizations to support and enhance wellness.
  • Answers and triages department calls, and distributes department referrals with guidance, in accordance with identified department service levels.
  • Collaborates and coordinates with other internal departments to provide a holistic approach to client care.
