JOB SUMMARY:

Update patient information (including Alternate Resource information). Assist patients in obtaining a State or Federal program for an Alternate Resource. Complete all forms through scheduled appointments and walk-ins. Provide exceptional patient service in all interactions. No supervisory responsibility.

9.Update patient file with personal information or Alternate Resource information by appointment or walk-in.
10.Update and/or confirm address, phone number, and alternate resource status, identifying eligibility status, and obtaining treatment authorization signature at each appointment.
11.Assist patients with the registration process for appointments or walk-ins.
12.Scan and retrieve registration documents.
13.Provide patients with all required medical forms including Government Performance and Results Act (GPRA), Staying Healthy Assessment (SHA), and Release of information (ROI)
14.Schedule appointments by telephone or in person, following established appointment time/diagnosis guidelines.
15.Schedule all Urgent Care (same day appointments).
16.Obtain information from patients and guide them to the appropriate department.
17.Verify and send appointments reminders daily through the Patient Portal Software.
18.Answer all telephone calls to the medical reception area promptly and courteously.
19. Screen and route all calls to the appropriate departments, taking messages to ensure accuracy and provide documentation of content.
20. Attend scheduled trainings and department meetings as requested by supervisor.
21. Ability to identify forms required for registration purposes and review for completion and accuracy.
22. Document, scan and maintain Medical Record information.
23. Comply with HIPAA regulations.

Behavioral Health Case Manager

The Redding Rancheria is seeking a Behavioral Health Case Manager. This position will be required to work at multiple facilities/work sites. The preferred schedule for this position will be Monday (10-hour shift) at our Trinity Health Center, Tuesday through Thursday (8:00am to 5:00pm) at the Center for Advanced Care, and Friday (6-hour shift; hours dependent upon if the employee takes a lunch) at the Center for Advanced Care.

JOB SUMMARY:

The Behavioral Health Case Manager will work directly with Behavioral Health patients and staff to effectively link patients to social services and treatment options. The Case Manager will help patients navigating the healthcare system by providing case management, care coordination, outreach, support and linkage to care.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Must have a Bachelor’s degree in psychology or social work.
  • Documented experience working in a mental health related field.
  • Preferred experience in evaluating patient eligibility and Alternate Resources status.
  • Must be proficient in using Microsoft Office Suite programs including Word, Excel, Publisher, and PowerPoint to make spreadsheets, graphs, manage data and create brochures, presentations and documents on computers as needed.

ESSENTIAL JOB DUTIES:

1. Provides case management services, referral and coordination with other allied professionals and treatment providers, including Medical Providers, Psychiatrists, Therapists, Tribal Child and Family Services, School IEP Coordinators, Courts, Substance Abuse Treatment Facilities, and other community-based agencies.
2. Initiates and coordinates individualized care plan for clients, addressing both clinical and non-clinical components, and ensuring the care plan is available to the patient, primary care provider, Behavioral Health Therapist, psychiatrist, substance abuse counselor and/or other internal departments as needed on a case-by-case basis.
3. Provides ongoing support for clients while continually assessing their need for increased mental health services and providing crisis intervention when needed.
4. Advocates for the patient/family to ensure the most compassionate and proper care is given.
5. Gathers patient records and any other necessary documentation from external facilities for providers to use for treatment planning.
6. Coordinates treatment planning with the Recovery Department and Tribal Court as needed.
7. Ensures smooth implementation and continuum, of care via effective and frequent communication with clients, or their identified health care designee(s) medical team members, and pharmacy technicians.
8. Coordinates referrals for services required to improve the client’s health status.
9. Maintains accurate and timely documentation, records, and case files in the NextGen system.
10. Manages and communicates lab results, referrals, and medication issues between providers and patients via clinical tasking.
11. Ensures follow up with patients regarding missed appointments and medication requests.
12. Rooms patients and records vital signs.
13. Reviews patient daily schedule for controlled medications and performs urine screenings for patients who are on controlled medications per DEA guidelines.
14. Completes prior authorizations and appeals for patient medications.
15. Develops and maintains knowledge of a community-based network of alternative modes of care; aids clients to connect with community-based organizations to support and enhance wellness.
16. Answers and triages department calls, and distributes department referrals with guidance, in accordance with identified department service levels.
17. Collaborates and coordinates with other internal departments to provide a holistic approach to client care.
18. Build and maintains positive working relationships with coworkers/team members, employees of other offices and departments within Redding Rancheria Tribal Health systems and community, using principals of good customer service.
19. Maintains patient confidentiality and protects operations by respecting and maintaining the privacy and dignity of patients, assuring client confidentiality at all times.

2. Responsible for the intake of patients for appointments, which includes verification of check-in procedures, review of medical screenings (depression, domestic violence), timely rooming, obtaining vital signs of patients (temperature, pulse, respiration, blood pressure, pain levels, oxygen saturations) ; confirms chief complaints/reason for visit ; reviews patient medical history, allergies, medications, and confirms information in Electronic Health Record (EHR).
3. Anticipates medical provider’s needs, and is proficient in set-up of medical procedures, including but not limited to, performing and/or preparing ordered lab specimens, procedural tray set-up for vasectomies, circumcisions, IUD placement/removals, sutures, casts, pap and pelvic examinations.
4. Responsible for data entry of patient information for compliance with quality measures (Quality Improvement Process (QIP); Government Performance and Results Act (GPRA), and Partnership Measures (as applicable) for facility reimbursement.
5. Assists with patient flow management- acts as a liaison to inform patients of current status, wait time, relays pertinent information to providers and is responsible to facilitate follow-up scheduling of appointments and/or rescheduling as needed.
6. Participates in patient education, ensuring the patient is provided resources to enhance understanding of medications, treatments, disease management, expectations and when to follow-up if needed. Assist with providing explanation of treatments and procedures at the start of applicable appointments.
7. Proficient and responsible for the delivery of routine immunizations, including the ability to perform rights of medication administration (right drug, dose, route, reason, patient, time, documentation), the ability to identify appropriate injection sites (adult / well-child); and adhere to antiseptic technique with medication preparation and delivery.
