Out of Area Differential Pay is available for this position to incentivize qualified staff to commute from a longer distance. JOB SUMMARY:

To initiate and coordinate a multidisciplinary team approach. Responsible for the delivery of appropriate, timely and beneficial care for Redding Rancheria Tribal Heath Systems patients. Alongside the patient care team, collaborates, plans, facilitates, evaluates and advocates to meet the comprehensive medical needs of the patient.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  1. Graduate of accredited school of licensed vocational nursing and possess current/valid LVN License.
  2. Familiarity with computer software preferred ex: Electronic Health Records
  3. CPR certification.

ESSENTIAL JOB DUTIES:

1. Ensures a smooth implementation and continuum of care via effective and frequent communication with care team members, patients, and identified health care designee.

11. Render assistance to the clinic providers and perform specified duties

12. Maintain compliance with applicable laws consistent with clinic policies and procedures

13. Advocates for the patient/family to ensure the most compassionate and proper care is given.

14. Manage panel for chronic care needs as well as health maintenance.

15. Responsible for daily oversight of Medical Assistants, which includes daily job performance, assisting Nursing Supervisor with employee evaluations and completion of        patient care and department assignments.

Purchase Referred Care Specialist

JOB SUMMARY:

Monitor referral and emergency room purchase orders pertaining to medical and dental services being provided under contract with providers outside the Redding Rancheria Tribal Health Center.  Ensure that services provided are delivered in accordance with established policies and procedures and contract.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  1. College course work in Accounting or related field or 3 years equivalent work experience.
  2. Demonstrated work experience in accounting or related area.
  3. Requires attention to detail involved in auditing financial statements.

ESSENTIAL JOB DUTIES:

  1. Audit emergency room visits for accuracy of 72-hour notice, eligibility for treatment and payments by alternate resources. An alternate resource is a payer that is fully or partially responsible for payment. Purchase Referred Care (PCR) is a payer of last resort. The Specialist must make every effort to obtain timely payment from all alternate resources before obligating funds from Purchase Referred Specialist.
  2. Inform the Registration/Alternate Resources Department when patients are referred to the Clinic without any insurance.
  3. Assist with payments to outside providers for services rendered for patients who have been determined to be eligible for PCR services. No payments may be made of funds obligated unless the PCR Committee has determined that the patient is eligible for services. Payments must be made on a timely basis since failure to do so jeopardizes critically important relationships with outside providers and delay in payments may result in loss of discounts to the Clinic. Payments consist of purchase orders made payable to outside providers.
  4. Provide assistance to outside providers as needed to track purchase orders, payments and eligibility for payment to those providers.
  5. Interact with patients and their families to ensure timely, accurate and complete receipt of bills, invoices, medical records and other documentation related to the care of patients. The Specialist must be familiar with and observe the guidelines of the Health Insurance Portability and Accountability Act (HIPAA) as they relate to Protected Health Information. The Specialist must follow the HIPAA guidelines when releasing Protected Health Information to anyone.
  6. For audit purposes, ensure that proper records are maintained. Records include verification of status as a California Indian, medical records, invoices, statements, explanation of benefit (EOB) forms, and any other documentation necessary to a complete understanding of why payment was rendered of denied. These records will be kept as a scanned document in Doc star.
  7. Maintain cost containment procedures as outlined in Clinic or Tribal policies or as directed by the Health Operations Director.
  8. Obligating funds in accordance with referral purchase orders that have been approved by the PRC Committee and preparing those referral purchase orders as they are approved by the PRC  Committee.   The Specialist must verify that each referral purchase order contains the appropriate signatures and authorizations from the PRC Committee.
  9. This position has no supervisory responsibilities.
  10. Maintain a good working relationship within the Clinic and Tribal Administration offices.  The Specialist will make every effort to accommodate the needs of other departments as they request assistance from PRC.
  11. Will assist in the negotiation of favorable reimbursement rates from PRC providers and attempt to maximize discounts available to the Clinic from the PRC providers.

10. Track key program metrics (no-show rates, caseload distribution, service utilization, etc.) and provide trend reports and recommendations to leadership.

11. Assist in Behavioral Health job description and policy and procedure development and/or revisions.

12. Lead or support special projects, audits, and performance improvement initiatives as requested by leadership.

Clinical Case Management and Direct Service:

13. Provide comprehensive case management across medical, behavioral health, educational, legal, and community systems to ensure coordinated and culturally responsive care.

14. Conduct mental-health assessments, provide crisis intervention, and support stabilization to reduce escalation and promote safety during mental health crisis.

15. Manage treatment funding budget and funding authorizations for Native American patients and tribal members, coordinating with the Recovery Department, Tribal Court, and external treatment facilities.

16. Facilitate safe care transitions through proactive communication with patients, families, pharmacies, providers, and cross-functional teams.

17. Coordinate referrals to internal and external services that support recovery, wellness, and continuity of care.

18. Maintain accurate, timely, and compliant documentation within NextGen EHR to support quality care, billing accuracy, and regulatory requirements.

19. Oversee lab results, medication issues, and referral updates, ensuring timely communication between patients and providers through clinical tasking workflows.

20. Conduct timely follow-up on missed appointments, medication requests, and ongoing patient needs to reduce care gaps and treatment adherence.

21. Collect patient histories, obtain routine vital signs, and document clinical information to support diagnostic and treatment planning.

22. Provide telephone triage, assess urgency, and direct patients to the appropriate level of care.

23. Perform urine drug screenings in compliance with DEA and chain-of-custody standards.

24. Manage prior authorizations for DME and specialty medications, ensuring accuracy, timely follow-up, and support for appeals.

25. Verify insurance eligibility for all scheduled patients to ensure accurate coverage and reduce financial barriers.

JOB SUMMARY:

The Medical Records & Referrals Supervisor oversees the day-to-day operations of the medical records and referrals departments, ensuring accurate documentation, timely processing, and compliance with state, federal, and organizational standards. This role is responsible for supervising staff, optimizing workflow efficiency, and supporting high-quality patient care through coordinated information management and referral processes.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Experience as a team leader.
  • 3–5 years of experience in medical records, health information management, or referral coordination
  • Healthcare related degree (i.e. Health Information Management, Health Informatics, Health Administration) preferred.

b. Support clinical teams by ensuring referral documentation, authorizations, and follow-ups are handled efficiently.

c. Collaborate with insurance companies to verify benefits, obtain authorizations, and resolve discrepancies or denials.

d. Track referral statuses, communicate updates to providers, and troubleshoot delays or barriers in care coordination.

e. Ensure the “referral loop” is closed for continuity of care.

4. Collaborate with clinical teams, billing, and administration to streamline workflows and resolve issues.

5. Ensure strict adherence to HIPAA and organizational privacy/security standards.

6. Identify gaps in workflow and implement corrective actions or process improvements.

7. Support accreditation requirements relevant to records management and referral coordination.

ESSENTIAL JOB DUTIES:

  1. Works as a collaborative part of the healthcare team to assist with initial admission of patients to the Health Center. Tasks include, but not limited to, new patient intake/data entry, processing of release of information (ROI), providing support and guidance to medical reception staff and coordination with medical records, gathering health information for the medical providers.
  2. Responsible for the intake of patients for appointments, which includes verification of check-in procedures, review of medical screenings (depression, domestic violence), timely rooming, obtaining vital signs of patients (temperature, pulse, respiration, blood pressure, pain levels, oxygen saturations) ; confirms chief complaints/reason for visit ; reviews patient medical history, allergies, medications, and confirms information in Electronic Health Record (EHR).
  3. Anticipates medical provider’s needs, and is proficient in set-up of medical procedures, including but not limited to, performing and/or preparing ordered lab specimens, procedural tray set-up for vasectomies, circumcisions, IUD placement/removals, sutures, casts, pap and pelvic examinations.
  4. Responsible for data entry of patient information for compliance with quality measures (Quality Improvement Process (QIP); Government Performance and Results Act (GPRA), and Partnership Measures (as applicable) for facility reimbursement.
  5. Assists with patient flow management- acts as a liaison to inform patients of current status, wait time, relays pertinent information to providers and is responsible to facilitate follow-up scheduling of appointments and/or rescheduling as needed.
  6. Participates in patient education, ensuring the patient is provided resources to enhance understanding of medications, treatments, disease management, expectations and when to follow-up if needed. Assist with providing explanation of treatments and procedures at the start of applicable appointments.
  7. Proficient and responsible for the delivery of routine immunizations, including the ability to perform rights of medication administration (right drug, dose, route, reason, patient, time, documentation), the ability to identify appropriate injection sites (adult / well-child; and adhere to antiseptic technique with medication preparation and delivery.
