JOB SUMMARY:

The caseworker will assist and provide Indian clients comprehensive community, social, and pro-family services to promote stability in Indian households. The Caseworker will also provide direct services to at-risk families as well as families ordered by the Tribal Court into reunification services or services designed to prevent out-of-home placement of an Indian Child. The Caseworker will also recruit and facilitate appropriate foster homes from Tribal Member children in need of care.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • BA degree preferred and/or at least 3-5 year work experience in this field.
  • Three years current work experience in Indian related community social services functions desired.
  • California Driver’s license required.
  • This position involves regular contact and/or control over minor children, as a result, a person who has been found criminally or civilly responsible for the abuse or neglect of a child, by any court, will not be eligible for employment in this position
  • Must be willing to submit to background check and fingerprinting.
  • Must pass pre-employment drug screening.

ESSENTIAL JOB FUNCTIONS:

  • Caseworker will receive complaints or referrals of at-risk youth, at risk families, and allegedly abused or neglected children and shall, with the consultation and under the direction of the
  • Tribal Social Worker and Community Services Manager, shall investigate all such referrals and make the appropriate reports. Further, will assist in the coordination with tribal and county staff to ensure a proper investigation and where necessary, coordinate with the Office of Tribal Attorney to file a petition with the court for a Child In Need of Care.
  • Will assist Indian children and their parents who are in child custody proceedings and placements to locate and obtain any needed services as ordered by the Court or directed by the Tribal Social Worker, including assisting by helping to locate appropriate housing, transportation, service providers or other necessary resources that contribute to child safety and planning.
  • Will assist in locating, certifying, and supervising foster, relative or other tribal child care placements.
  • The Caseworker shall be responsible for identifying interested parties and organizing Family Unity Conferences, including sending out notice of such conferences, in each case at regular intervals to insure family input and participation in case decision making.
  • Participate in multi-disciplinary teams and staff meetings.
  • Provides linkage, referral, advocacy, and service planning and crisis support as needed.
  • Monitors compliance with reunification plans of each case and consults with Social Worker and Community Services Manager on client compliance with service plans.
  • Monitors visitation with parents when children are placed out of the home under guidelines established by the department or the Social Worker or the Court.
  • Caseworker is required to appear in tribal court to testify on case status, develop and submit case plans under direction of Social Worker and make Department recommendations.
  • Assists Social Worker with Home Studies for prospective placements as well as routine home visits.
  • Adhere to the policy and procedures of the Family and Child Services.
  • Provide other duties and responsibilities as needed.

Acquire pertinent records, labs, x-rays, diagnostic tests and health summaries related to the patient’s current condition from the medical records; forward information to the specialist or facility involved.
  • Instruct and prepare patient for diagnostic procedures per clinic protocol or as instructed by provider.

  • Track all referrals sent out to ensure completion of care and receipt of related documentation. All data to be entered into the i2i system and regular reports to be generated for review by the Health Operations Director.

  • Respond to authorized requests for information by outside healthcare facilities
  • Receive outside records and scan to appropriate EHR patient file.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Graduate of accredited school of registered nursing and possess current/valid R.N. license.
  • Familiarity with computer software preferred ex: Electronic Health Records
  • CPR certification.
  • Must be fingerprinted and pass background check. Must pass pre-employment drug screening.
  • Indian preference.

ESSENTIAL JOB FUNCTIONS:

  • Admit patient to clinic, including initial triage and gathering health information.
  • Take patient history and routine vital signs of patients, including temperature, pulse, respiration, and blood pressure; and record results in patient charts.
  • Assist provider with medical procedures such as sutures, casts and examinations. Change dressings and apply bandages under the direction of the provider.
  • Perform routine tests consistent with training and clinic policies (i.e., audiometry, EKG, ultrasound, oximetry, spirometry, breathing treatments, visual acuity).
  • Assist provider in collecting of handling needed specimens (i.e. blood, urine) and prepare specimens for transfer and analysis at outside laboratory.

Physical Therapy Aide – Trinity

The physical therapy aide position includes considerable clerical duties and clinic up-keep responsibilities. The physical therapist aide must have physical stamina, patience, and patient focus to work effectively under the direction of the physical therapist to help patients regain and optimize movement after injury, illness, or medical procedures. The PT Aide supports physical therapy care delivery by assisting with tasks related to patient care, clerical responsibilities, and clinic safety.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

*  Moderate level of computer literacy
*  Knowledge of basic anatomy and medical terminology

ESSENTIAL JOB DUTIES:
* Use medical software to schedule patients and assist with maintaining patient records.
* Assist with management of patient referrals for physical therapy.
* Submit requests for Partnership authorization of therapy via “TAR.” Maintain record of TAR submissions and current authorizations.
* Build, maintain, and file patient paper charts.
*Communicate with other clinical staff regarding patient scheduling and other needs; and taking and delivering messages.
* Prepare clinic for patients by creating and maintaining a clean, sanitized, safe, treatment-ready environment for therapy-related activities.
* Make initial contact with therapy patients, welcome and assist into PT clinic, answer questions and requests, and allaying fears.
* Assist in gathering patient basic health information for the physical therapist.
* Instruct, motivate, safeguard and assist patients practicing exercises and functional activities under direction and direct supervision of physical therapist.
* Educate patients by demonstrating proper use of equipment and exercise routines.
*Assist patient with compiling and printing patient home exercise programs.
* Administer treatment modalities such as heat, ice, wax bath, ultrasound, and electrical modalities under direct supervision of physical therapist.
* Observe patients during treatment to compile and evaluate data on patients’ responses and progress, and report to physical therapist.
* In completion of patient visit, secure scheduling and escort (if necessary) from clinic.
* Confer with physical therapist to discuss patient information for planning, modifying, and coordinating treatment.
* Ensure operation of physical therapy equipment by completing preventive maintenance requirements following manufacturer’s instructions, troubleshooting malfunctioning equipment, and requesting repairs.
