JOB SUMMARY:

To initiate and coordinate a multidisciplinary team approach. Responsible for the delivery of appropriate, timely and beneficial care for RRTHC patients. Alongside the patient care team, collaborates, assesses, plans, facilitates, evaluates and advocates to meet the comprehensive medical needs of the patient.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Graduate of accredited school of registered nursing and possess current/valid RN license.
  • Familiarity with computer software preferred ex: Electronic Health Records

ESSENTIAL JOB FUNCTIONS:

1.Ensures a smooth implementation and continuum of care via effective and frequent communication with care team members, patients, and identified health care designee.

4.Maintain accurate and timely documentation in NextGen EHR.
5.Conduct comprehensive assessment of patient’s physical, mental and psychosocial needs.
6.Works with care team members during huddle times to coordinate daily patient care schedule as well as same day appointments. And double bookings as appropriate.
7.Manages assigned provider’s PAQ and task box.
8.Manage and communicate to patients verbally or by telephone (ie: lab results, referrals, x-rays, medication issues per provider task). Answers and triages department calls.
9.Actively participates in essential skills training.
10.Be able to perform or assist care team members in duties related to patient care (ie; rooming patients, vital signs, HPI, point of care testing).
11.Coordinate and assign duties to ma as needed to ensure all patient care needs are addressed and completed.
12.Render assistance to the clinic providers and perform specified duties.
13.Maintain compliance with applicable laws consistent with clinic policies and procedures.
14.Advocates for the patient/family to ensure the most compassionate and proper care is given.
15.Manage panel acute care and chronic care needs as well as health maintenance.
16.Responsible for daily oversight of medical assistants, which includes daily job performance and completion of patient care and department assignments.

Medical Assistant – RRTHC

JOB SUMMARY:

Under the direction of the assigned Nursing Supervisor and/or Director of Nursing, works as an integral part of the healthcare team, responsible for the facilitation and delivery of high quality, direct patient care.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Certification as Medical Assistant or 2 years of related healthcare experience or education related to medical assistant.
  • Knowledge of medical terminology/procedures, human anatomy and medical law/ethics.
  • CPR certification required.

ESSENTIAL JOB FUNCTIONS:

1. Works as a collaborative part of the healthcare team to assist with initial admission of patients to the Health Center. Tasks include, but not limited to, new patient intake/data entry, processing of release of information (ROI), providing support and guidance to medical reception staff and coordination with medical records, gathering health information for the medical providers.
2. Responsible for the intake of patients for appointments, which includes verification of check-in procedures, review of medical screenings (depression, domestic violence), timely rooming, obtaining vital signs of patients (temperature, pulse, respiration, blood pressure, pain levels, oxygen saturations) ; confirms chief complaints/reason for visit ; reviews patient medical history, allergies, medications, and confirms information in Electronic Health Record (EHR).
3. Anticipates medical provider’s needs, and is proficient in set-up of medical procedures, including but not limited to, performing and/or preparing ordered lab specimens, procedural tray set-up for vasectomies, circumcisions, IUD placement/removals, sutures, casts, pap and pelvic examinations.
4. Responsible for data entry of patient information for compliance with quality measures (Quality Improvement Process (QIP); Government Performance and Results Act (GPRA), and Partnership Measures (as applicable) for facility reimbursement.
5. Assists with patient flow management- acts as a liaison to inform patients of current status, wait time, relays pertinent information to providers and is responsible to facilitate follow-up scheduling of appointments and/or rescheduling as needed.
6. Participates in patient education, ensuring the patient is provided resources to enhance understanding of medications, treatments, disease management, expectations and when to follow-up if needed. Assist with providing explanation of treatments and procedures at the start of applicable appointments.
7. Proficient and responsible for the delivery of routine immunizations, including the ability to perform rights of medication administration (right drug, dose, route, reason, patient, time, documentation), the ability to identify appropriate injection sites (adult / well-child; and adhere to antiseptic technique with medication preparation and delivery.

JOB SUMMARY:

The Staffing and Resource Coordinator generates, manages, and updates employee master schedules and inventory management of clinical medical supplies. The position requires making adjustments as needed according to operational needs, staff call-ins, and pre-planned leaves of absence within the Tribal Healthcare System. In collaboration with the Director of Nursing and Nursing Supervisors, the Staffing and Resource Coordinator will coordinate staffing on a projected and shift by shift basis to ensure operational needs and necessary medical supplies are maintained across the health system.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • High School Diploma or Equivalent; Associate or Bachelor’s degree in logistics or inventory management preferred.
  • Staff scheduling experience in healthcare preferred, preferably in outpatient clinic setting.
  • Word processing, Excel, and spreadsheet and/or inventory software experience required.
  • Critical thinker with a problem-solving mind.
  • Previous experience in a warehouse or medical supply inventory assignments/roles.

11. Works with Supervisor staff when scheduling modified work duty employees in accordance with work restrictions and Rancheria policy.
12. Develops and maintains a good working rapport with interdepartmental personnel, as well as other departments within the facility.
13. Ensures administrative functions are carried out promptly for an efficient operation. Assists in standardizing the methods in which work will be accomplished.
14. Oversee the ordering, storing, receiving and distributing process of the health clinics product’s and supplies.
15. Evaluate the selection and shipping process and work to improve medical inventory to improve efficiency.
16. Keep up-to-date industry trends and update inventory as needed.
17. Oversee and manage the inventory storage space(s) to ensure appropriate room to store new product shipments.
18. Develop and maintain clinic par levels- restock orders and make sure essential inventory items are available.
19. Utilize inventory management software systems and technologies to manage and update inventory records accordingly.

Behavioral Health Lead Case Manager

JOB SUMMARY:

The Behavioral Health Lead Case Manager will be responsible for training new case managers and providing daily oversight to the Case Management Team in conjunction with the Behavioral Health Director.   2. Coordinates and assigns duties to Case Managers as needed to ensure all patient care needs are addressed and completed.

  8. Provides ongoing support for clients while continually assessing their need for increased mental health services and providing crisis intervention when needed.
  9. Advocates for the patient/family to ensure the most compassionate and proper care is given.
  10. Gathers patient records and any other necessary documentation from external facilities for
    providers to use for treatment planning.
  11. Maintains record of Substance Abuse Treatment funding budget for Native American patients and coordinates treatment planning with the Recovery Department and Tribal Court.
  12. Works with the Tribal Court team to coordinate and schedule psychological appointments with the psychologist(s) and scores assessments as needed.
  13. Ensures smooth implementation and continuum of care via effective and frequent communication with clients or their identified health care designee(s), medical team members, and pharmacy technicians.
  14. Coordinates referrals for services required to improve the client’s health status.
  15. Maintains accurate and timely documentation, records, and case files in the NextGen system.
  16. Manages and communicates lab results, referrals, and medication issues between providers and patients via clinical tasking.
  17. Ensures follow up with patients regarding missed appointments and medication requests.
  18. Rooms patients and records vital signs.
  19. Reviews patient daily schedule for controlled medications and performs urine screenings for patients who are on controlled medications per DEA guidelines

  21. Develops and maintains knowledge of a community-based network of alternative modes of care; aids clients to connect with community-based organizations to support and enhance wellness.
  22. Answers and triages department calls, and distributes department referrals with guidance, in accordance with identified department service levels.

  1. Primary liaison for the specialty care team, ensuring a smooth implementation and continuum of care through effective communication with care team members, patients, and health care designees.
