The Redding Rancheria is seeking a Behavioral Health Case Manager. This position will be required to work at multiple facilities/work sites. The preferred schedule for this position will be Monday (10-hour shift) at our Trinity Health Center, Tuesday through Thursday (8:00am to 5:00pm) at the Center for Advanced Care, and Friday (6-hour shift; hours dependent upon if the employee takes a lunch) at the Center for Advanced Care.

JOB SUMMARY:

The Behavioral Health Case Manager will work directly with Behavioral Health patients and staff to effectively link patients to social services and treatment options. ESSENTIAL JOB DUTIES:

1. Provides case management services, referral and coordination with other allied professionals and treatment providers, including Medical Providers, Psychiatrists, Therapists, Tribal Child and Family Services, School IEP Coordinators, Courts, Substance Abuse Treatment Facilities, and other community-based agencies.
2. Initiates and coordinates individualized care plan for clients, addressing both clinical and non-clinical components, and ensuring the care plan is available to the patient, primary care provider, Behavioral Health Therapist, psychiatrist, substance abuse counselor and/or other internal departments as needed on a case-by-case basis.
3. Provides ongoing support for clients while continually assessing their need for increased mental health services and providing crisis intervention when needed.
4. Advocates for the patient/family to ensure the most compassionate and proper care is given.
5. Gathers patient records and any other necessary documentation from external facilities for providers to use for treatment planning.
6. Coordinates treatment planning with the Recovery Department and Tribal Court as needed.
7. Ensures smooth implementation and continuum, of care via effective and frequent communication with clients, or their identified health care designee(s) medical team members, and pharmacy technicians.
8. Coordinates referrals for services required to improve the client’s health status.
9. Maintains accurate and timely documentation, records, and case files in the NextGen system.
10. Manages and communicates lab results, referrals, and medication issues between providers and patients via clinical tasking.
11. Ensures follow up with patients regarding missed appointments and medication requests.
12. Rooms patients and records vital signs.
13. Reviews patient daily schedule for controlled medications and performs urine screenings for patients who are on controlled medications per DEA guidelines.
14. Completes prior authorizations and appeals for patient medications.
15. Develops and maintains knowledge of a community-based network of alternative modes of care; aids clients to connect with community-based organizations to support and enhance wellness.
16. Answers and triages department calls, and distributes department referrals with guidance, in accordance with identified department service levels.
17. Collaborates and coordinates with other internal departments to provide a holistic approach to client care.
18. Build and maintains positive working relationships with coworkers/team members, employees of other offices and departments within Redding Rancheria Tribal Health systems and community, using principals of good customer service.
19. Maintains patient confidentiality and protects operations by respecting and maintaining the privacy and dignity of patients, assuring client confidentiality at all times.
20. Ensures insurance information is up-to-date and verifies client eligibility by running insurance information for each scheduled client.
21. Performs other related duties as assigned. Contributes to team effort by accomplishing related results as needed.

JOB SUMMARY:

This position is under the supervision of the Behavioral Health Director.

  • A Master’s degree in Marriage and Family Therapy, or a Master’s degree in Social Work
  • Must be currently licensed in good standing to practice as an independent Marriage and Family Therapist, or independent LCSW

  • Initiates and coordinates individualized care plan for clients, addressing both clinical and non-clinical components, and ensuring the care plan is available to the patient, primary care provider, Behavorial Health Therapist, psychiatrist, substance abuse counselor and/or other internal departments as needed on a case-by-case basis.
  • Provides ongoing support for clients while continually assessing their need for increased mental health services and providing crisis intervention when needed.
  • Advocates for the patient/family to ensure the most compassionate and proper care is given.
  • Gathers patient records and any other necessary documentation from external facilities for providers to use for treatment planning.
  • Coordinates treatment planning with the Recovery Department and Tribal Court as needed.
  • Ensures smooth implementation and continuum, of care via effective and frequent communication with clients or their identified health care designee(s), medical team members, and pharmacy technicians.
  • Coordinates referrals for services required to improve the client’s health status.
  • Maintains accurate and timely documentation, records, and case files in the NextGen system.
  • Manages and communicates lab results, referrals, and medication issues between providers and patients via clinical tasking.

  • Rooms patients and records vital signs.
  • Reviews patient daily schedule for controlled medications and performs urine screenings for patients who are on controlled medications per DEA guidelines
  • Completes prior authorizations and appeals for patient medications.
  • Develops and maintains knowledge of a community-based network of alternative modes of care; aids clients to connect with community-based organizations to support and enhance wellness.
  • Answers and triages department calls, and distributes department referrals with guidance, in accordance with identified department service levels.
  • Collaborates and coordinates with other internal departments to provide a holistic approach to client care.
  • Build and maintains positive working relationships with coworkers/team members, employees of other offices and departments within Redding Rancheria Tribal Health systems and community, using principals of good customer service.

ESSENTIAL JOB FUNCTIONS:

  • Provide mental health and substance abuse assessments, develop and implement treatment plans, provide case management, referral and coordination with other allied professionals and treatment providers, including medical providers, psychiatrists, Tribal Child and Family Services, school IEP coordinators, courts, and other community based agencies.
  • Have a functional understanding of DSM-5 psychiatric disorders, psychoactive medications, brain physiology and demonstrate a working knowledge of various theoretical models of psychotherapy, including brief therapy, behavioral counseling, substance abuse and other culturally relevant forms of treatment.
  • Facilitate and co-facilitate group therapy sessions for targeted client groups, such as adolescents, elders, trauma or abuse survivors, addictive behaviors, anger management, grief and/or support groups. Work with community to effectively advertise and recruit for group services.
  • Maintain electronic and other records in accordance with requirements of state, tribal, federal laws and clinic policy. Accurately complete all assessments, daily progress notes and client file documents within the required timeframe and provide them to the clinic, courts, probation and referral sources as appropriate and within HIPAA regulations.
  • Provide interpersonal support and problem solving assistance to individuals and families in need of services. Provide prevention, relapse prevention, education and follow-up information on mental health and substance abuse to individuals and families in our service area.
  • Provide consultation, training and technical assistance as requested to other departments and tribal entities. Assist with organizing staff and community activities that help the tribe respond to local human service and behavioral health needs in the interest of efficiency and responsiveness of services.
  • Respond promptly to requests from staff for support in crisis situations within the clinic or the tribal community. Practice appropriate crisis intervention skills. In conjunction with other members of the Behavioral Health Department and local human service providers, develop preparedness plans for responding to crisis situations within Shasta County. Refer individuals for follow-up as needed.
  • Participate as a team member of Redding Rancheria by actively participating in tribal and clinic meetings, and collaborating with varied departments to facilitate the most effective provision of services to beneficiaries. Develop a positive working environment by treating clinic and tribal staff members with respect and courtesy. Exercise sound judgment with problem solving and conflict resolution skills. Encourage others to achieve high levels of quality and successful working relationships. Promote team effectiveness, promoting trust, commitment and tribal pride.
  • Participate in supervision with the Behavioral Health Director to maintain quality of care and services, as needed. Participate in weekly Behavioral Health Department staff meetings/group supervision. Evaluate progress in an ongoing manner to include evaluation of employee’s own issues, such as counter-transference.
  • Possess and seek an ongoing fundamental understanding of Native American culture and local Native American communities. Maintain high quality work and patient care and provision of all services in a culturally competent and sensitive manner.
  • Obtain and/or maintain in good standing professional license and comply with continuing education requirements per applicable California licensing authority.
  • Proactively problem solve with supervisor appropriate time management and logistic strategies for working hours. Assist with program evaluations as requested.
