JOB SUMMARY:

The Computer Technician’s role is to support and maintain in-house computer systems, desktops, and peripherals. This includes installing, diagnosing, repairing, maintaining, and upgrading all hardware and equipment while ensuring optimal workstation performance. This person also assists in maintaining all forms of telecommunications and Audio Visual Systems such as telephone, fax, voice mail, electronic mail, paging and cellular telephones.

ESSENTIAL JOB DUTIES:

Community Health Data Analyst

JOB SUMMARY:

Responsible for overseeing all grant data including data collection, analysis, reporting, validation, and maintenance.  Coordinates annual audits for IHS grant reporting.  Support Diabetes Program by targeting patients with advanced data analytics, developing health outcome reports for the entire diabetic patient population, and identifying trends in patient health data. Provides patient education, coordinates outreach activities, and performs a wide number of administrative duties.

  9. Coordinate and run monthly Focus Group meetings for diabetic patients. Ensure proper event setup, coordinate food for event, and purchase supplies as needed. Coordinate and assist with Community Health events such as Mildred Rhoades Diabetes Prevention Walk, Kid’s Camp, and Annual Group Diabetic Clinics. Conduct patient outreach at aforementioned events to advertise for the services and classes offered by Community Health.
  10. Assist in developing program and diabetes education materials.
  11. Perform diabetic retinal screenings as a Certified Retinal Photographer through EyePACS.
  12. Maintain certification and keep up-to-date on latest non-mydriatic retinal camera hardware and techniques.

Medication Transporter

  3. In collaboration with QI committee and under the direction of the Quality Improvement Manager develops, implements and monitors QI activities/campaigns for RRTHS.

ESSENTIAL JOB DUTIES:

1. Provides case management services, referral and coordination with other allied professionals and treatment providers, including Medical Providers, Psychiatrists, Therapists, Tribal Child and Family Services, School IEP Coordinators, Courts, Substance Abuse Treatment Facilities, and other community-based agencies.
2. Initiates and coordinates individualized care plan for clients, addressing both clinical and non-clinical components, and ensuring the care plan is available to the patient, primary care provider, Behavioral Health Therapist, psychiatrist, substance abuse counselor and/or other internal departments as needed on a case-by-case basis.
3. Provides ongoing support for clients while continually assessing their need for increased mental health services and providing crisis intervention when needed.
4. Advocates for the patient/family to ensure the most compassionate and proper care is given.
5. Gathers patient records and any other necessary documentation from external facilities for providers to use for treatment planning.
6. Coordinates treatment planning with the Recovery Department and Tribal Court as needed.
7. Ensures smooth implementation and continuum, of care via effective and frequent communication with clients, or their identified health care designee(s) medical team members, and pharmacy technicians.
8. Coordinates referrals for services required to improve the client’s health status.
9. Maintains accurate and timely documentation, records, and case files in the NextGen system.
10. Manages and communicates lab results, referrals, and medication issues between providers and patients via clinical tasking.
11. Ensures follow up with patients regarding missed appointments and medication requests.
12. Rooms patients and records vital signs.
13. Reviews patient daily schedule for controlled medications and performs urine screenings for patients who are on controlled medications per DEA guidelines
14. Completes prior authorizations and appeals for patient medications.
15. Develops and maintains knowledge of a community-based network of alternative modes of care; aids clients to connect with community-based organizations to support and enhance wellness.
16. Answers and triages department calls, and distributes department referrals with guidance, in accordance with identified department service levels.
17. Collaborates and coordinates with other internal departments to provide a holistic approach to client care.
18. Build and maintains positive working relationships with coworkers/team members, employees of other offices and departments within Redding Rancheria Tribal Health systems and community, using principals of good customer service.
19. Maintains patient confidentiality and protects operations by respecting and maintaining the privacy and dignity of patients, assuring client confidentiality at all times.
20. Ensures insurance information is up-to-date and verifies client eligibility by running insurance information for each scheduled client.
21. Performs other related duties as assigned. Contributes to team effort by accomplishing related results as needed.

JOB SUMMARY:

Under the direction of the Director of Program Services, incumbent is responsible for all programs under the Education department including: Head Start, Child Care, Youth Leadership and Enrichment Activities, The Scholarship Program, The Community Education Fund, Student Achievement, , Tutoring, Johnson O’Malley, and the School Clothing benefits program.. This includes staff supervision and the organization implementation of activities procedures that assure an efficient program and a safe environment for both children and staff. Responsible for fiscal and budgetary accountability and compliance with all state, federal and tribal guidelines and regulations. Secure program funding through the research of potential grant programs, tribal funding, and assist in writing grant proposals.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

1. This position requires a successful applicant to have control of, or significant contact with, a child or children. Therefore, federal law requires all applicants be subjected to a thorough criminal background investigation including a character assessment. The criminal background check will include state, federal, and tribal criminal justice reporting as well child protective services reporting. A criminal conviction, or an affirmed allegation in a child protective proceeding, of abuse or neglect of a child will disqualify the applicant from consideration for employment.

9. Coordinate the annual tribal Youth Christmas Party: purchase presents, raffle items, and mail out-of-town youth checks.

17. Knows and ensures compliance with CCFP and Tribal regulations pertaining to menu, meal counts, family documentation, etc.

18. Ensure and maintain a safe and caring environment for children and staff and that no child/adult is discriminated against by basis of religion, race, gender or ability.

19. Ensure that all staff adheres to the NAEYC Developmentally Appropriate Practices in Head Start and Childcare Programs.

20. Provide ongoing assistance and training in locating materials and planning age appropriate classroom or meeting activities with cultural relevance.

21. Ensure all Redding Rancheria Education Department programs maintain current and complete enrollment files for all participants including an up-to-date database with addresses and phone numbers for all adult and minor tribal members for the purposes of program eligibility, mailing and other communications.

22. Ensures the development and implementation of a Parent Education program; topics to include health, nutrition, parenting, social services, kindergarten, transitions to schools, parent child relations, etc.

23. Responsible for cultivating and maintaining a positive professional working relationship with parents, and other family members and/or childcare givers. Encourage and facilitate regular two-way communication. (with all education programs)

24. Ensure that relevant councils and committees are organized and facilitated. Serve as staff support as needed. Provide in-service training as needed regarding their roles, responsibilities, and authority.

25. As mandated, identify and will report suspected child abuse to proper authorities following established guidelines. Responsible for ensures safety of children at all times.

26. Plans, organized, directs and evaluates the performance of assigned staff; establishes performance requirements and personal development targets; makes decisions on compensation and other rewards to recognize performance; takes disciplinary action, up to and including termination, to address performance deficiencies, in accordance with Redding Rancheria personnel rules and policies.
