JOB SUMMARY:

The Community Health Worker will be responsible for acting as a bridge between healthcare providers and the community to facilitate access to services and improve the quality and cultural competence of service delivery. This role will focus on community outreach, preventive health education, basic diagnostic procedures, and patient monitoring.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  1. High school diploma or equivalent required; some college or vocational training preferred.
  2. One year of experience working in a community-based setting preferred.
  3. Certified as a Community Health Worker or willingness to complete certification within the first year of employment.

Psychiatry Lead Case Manager

The Redding Rancheria is seeking a Psychiatry Lead Case Manager. The preferred schedule for this position is Monday through Friday, 8:00 AM to 5:00 PM.

JOB SUMMARY:

The Psychiatry Lead Case Manager will be responsible for training new case managers and providing daily oversight to the Case Management Team in conjunction with the Behavioral Health Director. Oversight includes but is not limited to, guidance related to service delivery and documentation, maintaining compliance with regulatory standards, appropriately managing caseloads, and coordinating work schedules. The Psychiatry Case Manager Lead will work directly with Behavioral Health patients and staff to effectively link patients to social services and treatment options. The Case Manager Lead will help patients navigating the healthcare system by providing case management, care coordination, outreach, support and linkage to care.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  1. Must have a Bachelor’s degree in psychology or social work.
  2. Documented experience working in a mental health related field.
  3. 4-years case management experience.

1. Provide comprehensive, end-to-end case management across medical, behavioral health, educational, legal, and community systems to ensure clients receive coordinated, effective, and culturally responsive care.

9. Manage and coordinate referrals to internal and external services that improve patient health, recovery outcomes, and overall wellness.

10. Maintain accurate, timely, and compliant documentation within the NextGen EHR to support regulatory requirements, billing accuracy, and high-quality clinical data.

11. Oversee lab results, medication issues, and referral updates, ensuring rapid communication between providers and patients through clinical tasking workflows.

12. Conduct timely follow-up for missed appointments, medication requests, and ongoing patient concerns to reduce care gaps and improve treatment adherence.

13. Collect detailed patient histories, obtain routine vital signs, and document findings to support clinical decision-making and diagnostic accuracy.

Medical Scribe

JOB SUMMARY:
Medical Scribes are individuals trained in medical documentation who assist a provider throughout their shift. The primary goal is to increase the efficiency and productivity of the physician. It will be the responsibility of the Medical Scribe to annotate any dictated or written information for the treatment of patients following all local, state and federal guidelines for documentation into the electronic health record system. The Medical Scribe will be responsible for clearly recording each patient’s symptoms, history, physical exam and documenting the diagnosis. This position will have no direct involvement in patient care but has an interdepartmental role in assisting nursing and front office staff.

• The primary role of the Medical Scribe is to assist the clinician with documentation of each patient’s medical chart during his or her visit.

The Clinical Data Analyst’s primary role is to gather data, conduct analysis and create reports or models as requested by various departments across the organization. The Clinical Data Analyst works collaboratively with key clinical and business leaders to develop data analysis solutions and visualizations to help optimize performance and to support strategic planning.

Requirements/Minimum Qualifications:

  • Bachelor’s degree (preferably in Health Informatics, Data Analytics or Computer Science) or equivalent experience
  • 3+ years of experience in the healthcare setting
  • Experience with relational databases with a SQL emphasis
  • Knowledge of relational database reporting tools (i.e. SSMS, SSRS, Access)
  • Knowledge of data analytics tools and statistics
  • Skilled in Microsoft including Word, Excel, PowerPoint and Outlook (experience with Excel macros and VBA desired)
