Redding Rancheria is seeking a LVN II- Care Coordinator. To initiate and coordinate a multidisciplinary team approach. Responsible for the delivery of appropriate, timely and beneficial care for Redding Rancheria Tribal Heath Systems patients. Alongside the patient care team, collaborates, plans, facilitates, evaluates and advocates to meet the comprehensive medical needs of the patient. Out of Area Differential Pay is available for this position to incentivize qualified staff to commute from a longer distance. This will allow Trinity Health Care patients to receive better quality care.

Requirements/Minimum Qualifications:

Graduate of accredited school of licensed vocational nursing and possess current/valid LVN license.
Familiarity with computer software preferred ex: Electronic Health Records
CPR certification.
Must be fingerprinted and pass background check.
