JOB SUMMARY:

The Medical Records & Referrals Supervisor oversees the day-to-day operations of the medical records and referrals departments, ensuring accurate documentation, timely processing, and compliance with state, federal, and organizational standards. This role is responsible for supervising staff, optimizing workflow efficiency, and supporting high-quality patient care through coordinated information management and referral processes.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Experience as a team leader.
  • 3–5 years of experience in medical records, health information management, or referral coordination
  • Healthcare related degree (i.e. Health Information Management, Health Informatics, Health Administration) preferred.

1. Supervisory and Leadership Duties:

a.Lead, train, and mentor medical records and referral staff, including hiring, performance evaluations, scheduling, and disciplinary actions

c. Manage daily operations, workflow, and productivity within the department.

d. Prepare reports and present departmental performance as needed to leadership.

2. Medical Records Management:

a. Ensure timely retrieval, release, scanning, indexing, and archiving of medical information.

b. Monitor data integrity and accuracy across electronic health records (EHR) systems

c. Manage requests for records, including internal/external providers, legal requests, and patient inquiries.

d. Conduct routine audits to ensure compliance, accuracy, and proper documentation standards.

3. Referrals Coordination:

a. Supervise referral processing workflows, ensuring referrals are completed accurately and within designated timeframes.

b. Support clinical teams by ensuring referral documentation, authorizations, and follow-ups are handled efficiently.

c. Collaborate with insurance companies to verify benefits, obtain authorizations, and resolve discrepancies or denials.

d. Track referral statuses, communicate updates to providers, and troubleshoot delays or barriers in care coordination.

e. Ensure the “referral loop” is closed for continuity of care.

4. Collaborate with clinical teams, billing, and administration to streamline workflows and resolve issues.

5. Ensure strict adherence to HIPAA and organizational privacy/security standards.

6. Identify gaps in workflow and implement corrective actions or process improvements.

7. Support accreditation requirements relevant to records management and referral coordination.

JOB SUMMARY:

Maintain and protect patient medical records in accordance with clinic policies and guidelines for patient confidentiality.

JOB SUMMARY:

A Clinical Systems Analyst is responsible for leading and participating in planning, development, implementation, training, evaluation of use, and optimization of clinical informatics systems. This role is responsible for building, testing and implementing computer applications and other systems that support Redding Rancheria Tribal Health System’s business and clinical objectives. The Clinical Systems Analyst will serve as the internal expert for all clinical systems and is responsible for assisting in long term maintenance and optimization activities.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Bachelor’s degree preferred or an equivalent combination of education and work experience.
  • Minimum four (4) years experience working with EPM and EHR applications, NextGen preferred.
  • Previous systems/application support and analysis in the healthcare industry, Nextgen preferred.
  • Completion of minimum two (2) year Health Information Technology training program or 3 years clinical systems experience.
  • Nextgen Certification (NCP) preferred or certified within 90 days of hire.
  • Experienced in relational and dimensional data modeling principles and techniques.
  • Certification in at least HL7 or SQL.
  • Experience as a lead analyst/tester on large scale Clinical Informatics projects.

Quality Improvement Specialist

JOB SUMMARY:

Under the direction of the Quality Improvement Manager works as part of a multi-disciplinary team to ensure Redding Rancheria Tribal Health System (RRTHS) patient population receives quality patient care. The QI coordinator is responsible for assisting in the coordination, integration, and implementation of the organization wide performance improvement activities to ensure compliance with company policies and state/federal regulatory and accreditation core standards.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  1. State RN licensure or Certification in a healthcare field – Bachelor’s Degree preferred.
  2. Healthcare Quality experience, working with IHS, AAAHC, Partnership, HEDIS and GPRA strongly desired.
  3. Experience working in a clinic setting and a demonstrated commitment to improving quality care for patients within RRTHS.
  4. Proficient in Word processing, spreadsheets, database and typing required.
  5. Experience manipulating and analyzing data with a focus on detail and organizational skills.

  3. In collaboration with QI committee and under the direction of the Quality Improvement Manager develops, implements and monitors QI activities/campaigns for RRTHS.

  5. Works with clinical staff to optimize use of the EHR to ensure data quality.
  6. Coordinates new and existing employee trainings with the training department and managers related to quality initiatives, under the direction of the QI Manager and/or assigned Director.

  8. Performs concurrent and retrospective case reviews.
  9. Conducts chart reviews, screenings and peer reviews to assess strengths, challenges and barriers to compliance, evaluates this data and determines best outreach strategy.

Provide patient education during outreach calls as it relates to quality improvement activities (which may include, but not limited to: Encourage compliance with preventative and chronic disease management services; provide reminders to patients on upcoming preventive health screening appointments; educate patients on the importance of routine health visits).
  11. Assist with QI team meetings- may include organizing meeting agendas, meeting minutes and other necessary documentation as assigned.

The Redding Rancheria is seeking a Medical Data & Referrals Specialist (CCHC). The schedule for this position is Monday through Friday, 8:00am-5:00pm.

JOB SUMMARY:

Coordinate referrals from Redding Rancheria Tribal Health Systems to other health care providers for patient care that is not performed at the Health Center. Referrals are made to providers such as physicians, diagnostic facilities, radiology, laboratories, and hospitals.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • High School diploma or GED required.
  • Experience with pre-authorization process for 3rd Party payers.
  • Medical Terminology.
  • Must pass pre-employment drug screening.
  • Must be fingerprinted and pass background check.
  • Indian preference.

ESSENTIAL JOB DUTIES:

1. Receive referrals/requests from providers through the EHR and schedule outside care as indicated.
2. Provide information to patients regarding referral provider name, address and phone number to allow patients to call to schedule appointment. Acquire pertinent records, labs, x-rays, diagnostic tests and health summaries related to the patient’s current condition from the medical records; forward information to the specialist or facility involved.
3. Enter referral data, and insurance information into EHR..
4. Track all referrals sent out to ensure completion of care and receipt of related documentation using SQL reports. .
5. Maintain all required data in the EHR to comply with clinic, state and federal mandates.
6. Request medical records/other additional information from outside healthcare facilities.
7. Respond to authorized requests for information by outside healthcare facilities
8. Receive outside records and scan to appropriate EHR patient file.
9. Scan all ‘in house’ forms and documents to the appropriate EHR patient file.
10. Track all stored historical paper charts and shred as per policy as they time out. Perform other related duties as needed.

JOB SUMMARY:

Maintain and protect patient medical records in accordance with clinic policies and guidelines for patient confidentiality.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Prefer medical records experience to include numerical chart filing, lab and x-ray disposition. Must have professional phone courtesy.

Quality Improvement Manager

JOB SUMMARY: Under the direction of the Director of Informatics the Quality Improvement Manager (QIM) plays a vital role in developing and managing the continuous quality improvement and quality assurance programs within the Redding Rancheria Tribal Health System (RRTHS). The Quality Improvement Manager manages the performance of health center quality improvement activities, provider quality reviews, establishes and monitors quality improvement goals, and organizes outcomes research. This position is an integral part of improving overall clinical quality measures and will oversee the development of clinical and operational reports; clinical compliance with HEIDS / GPRA measures and regulatory requirements to enhance patient outcomes and meet organizational goals.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  •  Bachelor’s Degree required, preferably in a healthcare related field.
  • Master’s Degree preferred.
  • State RN licensure or a License in healthcare field preferred.
  • 4 – 5 years’ healthcare experience. 1-4 years’ quality improvement experience.
  • Supervisory experience required.
  • Certified professional in healthcare quality preferred.
  • Healthcare Quality experience, working with IHS, AAAHC, Partnership, HEDIS and GPRA strongly desired.
  • Experience working in a clinic setting and a demonstrated commitment to improving quality care for patients within RRTHS.
  • Excellent knowledge of community health needs, data analysis skills, programs evaluation methods, program planning tools/methods, as well as public and community health models and frameworks.

  • Supervise the overall performance of staff in the Quality Improvement (QI) department.
  • Develop and Implement interventions to improve performance on measures. Works closely with medical operations on Quality Improvement efforts based on annual QI program and evaluations.
  • Becomes a subject matter expert (SME) in company quality initiatives, as well as the products, functions, and policy and procedures that support those initiatives.
  • Assist in the development of Quality Improvement initiatives in the clinic as identified by accreditation and/or regulatory agencies.
  • Oversee the generation of QI reports, track quality improvement activities and outcomes quarterly to the QI committee to ensure measurable improvements in clinical and operational outcomes and customer satisfaction.
  • Participate in Quality Improvement Committee meetings and QI work plans as needed.
  • Work with leadership to prepare policies/procedures to coordinate care management of patients, and reports that measure performance in various preventive clinical measures.
  • Prepare and manage application and submission process for accreditation and regulatory site review visits.
  • Monitor clinic workflow/documentation and make recommendations to the leadership team as to the integration of quality approach into daily operations.
  • Responsible for overseeing the production of all monthly, quarterly, final reports and applications to regulating agencies to ensure reports are accurate, and timely submitted.
  • Responsible for coordinating and managing health system wide performance improvement activities including continued survey readiness.
