JOB SUMMARY:

Update patient information (including Alternate Resource information). Assist patients in obtaining a State or Federal program for an Alternate Resource. Complete all forms through scheduled appointments and walk-ins. No supervisory responsibility.

JOB SUMMARY:

Performs a variety of general cleaning and custodial work.

LVN II Care Coordinator – Trinity

The Redding Rancheria is seeking an LVN II Care Coordinator. The preferred schedule for this position is Tuesday through Friday, 7:00am to 6:00pm.

Out of Area Differential Pay is available for this position to incentivize qualified staff to commute from a longer distance. JOB SUMMARY:

To initiate and coordinate a multidisciplinary team approach. Responsible for the delivery of appropriate, timely and beneficial care for Redding Rancheria Tribal Heath Systems patients. Alongside the patient care team, collaborates, plans, facilitates, evaluates and advocates to meet the comprehensive medical needs of the patient.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

ESSENTIAL JOB DUTIES:

1. Ensures a smooth implementation and continuum of care via effective and frequent communication with care team members, patients, and identified health care designee.

14. Manage panel for chronic care needs as well as health maintenance.

Education Specialist (Nursing)

JOB SUMMARY:

The Education Specialist supports the ongoing education, training, and competency development of nursing and clinical support staff within the Tribal Health System. Under the direction of the Director of Nursing and Nurse Educator, this role assists in planning, delivering, tracking, and providing training that promote high-quality patient care, regulatory compliance, and professional growth. The Education Specialist functions as a clinical resource, helps ensure consistency in practice standards, and contributes to the development of a positive learning environment across all clinical departments.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  1. National Certification as a Medical Assistant (e.g., AAMA, NHA, AMT) required.
  2. Minimum 5 years of experience working in a healthcare setting, preferably in an outpatient or Tribal Health Clinic.
  3. Proficient Electronic Health Record (EHR) experience required; preferably with NextGen.
  4. Experience in staff education, training, clinical precepting, or quality improvement preferred.
  5. Must possess a valid California driver’s license and have reliable transportation to and from work sites.
  6. Must be willing to work at other clinic sites to support training, onboarding, and departmental needs.
  7. Current BLS/CPR certification required.
  8. Indian preference.
  9. Strong communication skills and ability to teach diverse adult learners.
  10. Must meet Tribal Health System background check and pre-employment requirements.

ESSENTIAL JOB DUTIES:

1. Collaborate with the Nurse Educator to assess the training needs of nursing and clinical support staff and develop appropriate education plans.

2. Assist in planning, coordinating, and delivering educational sessions, skills labs, competency assessments, and orientation programs for new and existing staff.

3. Support creation and revision of training materials, competencies, checklists, workflows, and procedure guides.

4. Participate in onboarding and orientation processes for new hires, ensuring timely completion of required learning modules and 1:1 skills validation.

5. Provide direct preceptorship for new clinical hires, including hands-on skills training, workflow orientation, and competency validation.

6. Assist with monitoring and maintaining staff compliance with regulatory, accreditation, and organizational training requirements.

7. Provide hands-on clinical skills coaching, mentorship, and real-time support to clinical staff.

8. Help identify clinical practice gaps and work with the Nurse Educator and Director of Nursing to implement corrective training strategies.

9. Support documentation and tracking of all staff training, competencies, and continuing education.

10. Serve as an on-the-floor clinical resource, modelling best practices and supporting staff during procedures, patient care tasks, and daily clinic operations.

11. Maintain up-to-date knowledge of best practices in ambulatory care, infection control, patient safety, and Redding Rancheria Tribal Health System (RRTHS) standards.

12. Participate in quality improvement initiatives, chart audits, and workflow evaluations to identify education-related opportunities.

13. Observe clinical staff performance in real time and offer feedback to ensure safe and consistent practice amongst the health clinics.
