The Redding Rancheria is seeking a RN Care Coordinator – Specialty Services.
JOB SUMMARY:
To initiate and coordinate a multidisciplinary team approach in providing patient centered care. Responsible for the delivery of appropriate, timely and beneficial care for RRTHS patients. Alongside the specialty care team, collaborates, assesses, plans, facilitates, evaluates, and advocates to meet the comprehensive medical needs of the patient.
REQUIREMENTS/MINIMUM QUALIFICATIONS:
- Graduate of accredited school of registered nursing and possess current/valid RN license to practice as a nurse in the state of California.
- Experience in outpatient/clinic setting preferred.
- Familiarity with computer software preferred ex: Electronic Health Records preferred.
- CPR certification.
- Must be fingerprinted and pass background check.
- Must pass pre-employment drug screening.
- Indian preference.
ESSENTIAL JOB FUNCTIONS:
- Primary liaison for the specialty care team, ensuring a smooth implementation and continuum of care through effective communication with care team members, patients, and health care designees.
- Responsible for the review and management of assigned provider’s physical clinic inbox for documents requiring provider action and providing the necessary supportive interventions.
- Assists with the identification of time sensitive healthcare documents and appropriately navigates processes to ensure items are addressed.
- Responsible for the support and overview of Provider Approval Que (PAQ) in the electronic healthcare record (EHR); reviewing items in the nursing triage and individual inbox for assigned providers to take appropriate action and present issues to the provider in person or via telephone communication within the EHR.
- Anticipates medical provider’s schedule, and is proficient in appointment scheduling, including, but not limited to, review of referral, outreach to patient and/or community based referral sources.
- Provides notification and/or communication with patients verbally or by telephone regarding specialty services within the RN scope of practice.
- Participates in providing education, ensuring the patient is provided resources to enhance understanding of medications, treatments, disease management, expectations and when to follow-up if needed. Assist with providing explanation of treatments and procedures related to specialty support services.
- Responsible for routine monitoring and review of Practice Management (PM) electronic schedule to support workflow, advocate for work-ins, and management of provider patient schedules.
- Works collaboratively with other members of the specialty services to ensure timely responses to internal and external referrals for in-house specialty services, including but not limited to, cardiology, endocrinology, neurology, dermatology, rheumatology, and podiatry.
- Renders assistance with patient triage (physical and/or phone) to identify and support necessary medical interventions.
- Application of nursing skills as indicated, which may include, but not limited to admitting patient to clinic, initial triage, gathering health information (HPI), obtaining vital signs (temp, pulse, HR, O2, BP) and resulting in patient electronic chart.
- Must have the ability to assist providers and/or medical assistant staff with clinical tasks which may include, but not limited to, procedural set-up and support (sutures, casts, splints, wound care), ability to administer injections, perform routine tests (audiometry, visual acuity, EKG, spirometry, nebulizer treatments, etc.).
- Assists with the insertion and maintenance of intravenous therapy (IV) for patients within the clinic.
- Responsible for the routine management of in-house medication inventory and emergency cart procedures to ensure regulatory compliance and adherence to RRTHS policy and procedure.
- Actively participates in assigned trainings, recommends and supports continued education and clinical competencies as indicated.
- Relays pertinent feedback and observations to nursing supervisor for awareness and additional follow-up as indicated.
