JOB SUMMARY:

To initiate and coordinate a multidisciplinary team approach in providing patient centered care. Responsible for the delivery of appropriate, timely and beneficial care for RRTHS patients. Alongside the specialty care team, collaborates, assesses, plans, facilitates, evaluates, and advocates to meet the comprehensive medical needs of the patient.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Graduate of accredited school of registered nursing and possess current/valid RN license to practice as a nurse in the state of California.
  • Experience in outpatient/clinic setting preferred.

ESSENTIAL JOB FUNCTIONS:

  1. Primary liaison for the specialty care team, ensuring a smooth implementation and continuum of care through effective communication with care team members, patients, and health care designees.
  2. Responsible for the review and management of assigned provider’s physical clinic inbox for documents requiring provider action and providing the necessary supportive interventions.
  3. Assists with the identification of time sensitive healthcare documents and appropriately navigates processes to ensure items are addressed.
  4. Responsible for the support and overview of Provider Approval Que (PAQ) in the electronic healthcare record (EHR); reviewing items in the nursing triage and individual inbox for assigned providers to take appropriate action and present issues to the provider in person or via telephone communication within the EHR.

  7. Participates in providing education, ensuring the patient is provided resources to enhance understanding of medications, treatments, disease management, expectations and when to follow-up if needed.   8. Responsible for routine monitoring and review of Practice Management (PM) electronic schedule to support workflow, advocate for work-ins, and management of provider patient schedules.
  9. Works collaboratively with other members of the specialty services to ensure timely responses to internal and external referrals for in-house specialty services, including but not limited to, cardiology, endocrinology, neurology, dermatology, rheumatology, and podiatry.
  10. Renders assistance with patient triage (physical and/or phone) to identify and support necessary medical interventions.
  11. Application of nursing skills as indicated, which may include, but not limited to admitting patient to clinic, initial triage, gathering health information (HPI), obtaining vital signs (temp, pulse, HR, O2, BP) and resulting in patient electronic chart.
  12. Must have the ability to assist providers and/or medical assistant staff with clinical tasks which may include, but not limited to, procedural set-up and support (sutures, casts, splints, wound care), ability to administer injections, perform routine tests (audiometry, visual acuity, EKG, spirometry, nebulizer treatments, etc.).
  13. Assists with the insertion and maintenance of intravenous therapy (IV) for patients within the clinic.
  14. Responsible for the routine management of in-house medication inventory and emergency cart procedures to ensure regulatory compliance and adherence to RRTHS policy and procedure.
  15. Actively participates in assigned trainings, recommends and supports continued education and clinical competencies as indicated.
  16. Relays pertinent feedback and observations to nursing supervisor for awareness and additional follow-up as indicated.

JOB SUMMARY:

The Licensed Therapist is licensed/certified independently to provide direct Mental Health and Chemical Dependency services in an outpatient medical setting to Native Americans residing in the eastern half of Trinity County and the western two-thirds of Shasta county who have mental health, substance abuse, or co-occurring disorders. This provider will conduct mental health intakes, substance abuse assessments, individual, family and group treatment and Integrative/crisis services to patients and their families. Responsibilities also include serving as a consultant for program services to Redding Rancheria staff and community, and working to increase the understanding of individual and community behavioral health issues and service delivery. This position is under the supervision of the Behavioral Health Director.

Requirements/Minimum Qualifications:

  • A Master’s degree in Marriage and Family Therapy, or a Master’s degree in Social Work
  • Must be currently licensed in good standing to practice as an independent Marriage and Family Therapist, or independent LCSW
  • Must pass background assessment of moral character and legal status. Must submit to fingerprint identification with law enforcement and be able to work with children.

JOB SUMMARY:

To provide supportive case management services for patients within the Behavioral Health Department of the Redding Rancheria Tribal Health Center.

 

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  • Must have a Bachelor’s degree in psychology or social work.
  • Documented experience working in a mental health related field.
  • Preferred experience in evaluating patient eligibility and Alternate Resources status.
  • Must be proficient in using Microsoft Office Suite programs including Word, Excel, Publisher, and PowerPoint to make spreadsheets, graphs, manage data and create brochures, presentations and documents on computers as needed.
  • Must pass background assessment of moral character and legal status. Must submit to fingerprint identification with law enforcement and be able to work with children.

  • Be in possession of valid and current California Driver’s License.

ESSENTIAL JOB FUNCTIONS:

  • Provides case management services, referral and coordination with other allied professionals and treatment providers, including Medical Providers, Psychiatrists, Therapists, Tribal Child and Family Services, School IEP Coordinators, Courts, Substance Abuse Treatment Facilities, and other community-based agencies.
  • Initiates and coordinates individualized care plan for clients, addressing both clinical and non-clinical components, and ensuring the care plan is available to the patient, primary care provider, Behavorial Health Therapist, psychiatrist, substance abuse counselor and/or other internal departments as needed on a case-by-case basis.
  • Provides ongoing support for clients while continually assessing their need for increased mental health services and providing crisis intervention when needed.
  • Advocates for the patient/family to ensure the most compassionate and proper care is given.
  • Gathers patient records and any other necessary documentation from external facilities for providers to use for treatment planning.
  • Coordinates treatment planning with the Recovery Department and Tribal Court as needed.
  • Ensures smooth implementation and continuum, of care via effective and frequent communication with clients or their identified health care designee(s), medical team members, and pharmacy technicians.
  • Coordinates referrals for services required to improve the client’s health status.
  • Maintains accurate and timely documentation, records, and case files in the NextGen system.
  • Manages and communicates lab results, referrals, and medication issues between providers and patients via clinical tasking.
  • Ensures follow up with patients regarding missed appointments and medication requests.
  • Rooms patients and records vital signs.
  • Reviews patient daily schedule for controlled medications and performs urine screenings for patients who are on controlled medications per DEA guidelines
  • Completes prior authorizations and appeals for patient medications.
  • Develops and maintains knowledge of a community-based network of alternative modes of care; aids clients to connect with community-based organizations to support and enhance wellness.
  • Answers and triages department calls, and distributes department referrals with guidance, in accordance with identified department service levels.
  • Collaborates and coordinates with other internal departments to provide a holistic approach to client care.
