LVN Care Coordinator – Recovery

The Redding Rancheria is seeking a LVN Care Coordinator – Recovery. The preferred schedule for this position is Monday through Friday, 8:00am-5:00pm.

JOB SUMMARY:

To initiate and coordinate a multidisciplinary team approach. Responsible for the delivery of appropriate, timely and beneficial care for Redding Rancheria Tribal Heath Systems patients. Alongside the patient care team, collaborates, plans, facilitates, evaluates and advocates to meet the comprehensive medical needs of the patient.

REQUIREMENTS/MINIMUM QUALIFICATIONS:

  1. Graduate of accredited school of licensed vocational nursing and possess current/valid LVN License.
  2. Familiarity with computer software preferred ex: Electronic Health Records
  3. CPR certification.
  4. Must possess a valid California driver’s license and have reliable transportation to and from work sites.
  5. Must be fingerprinted and pass background check.
  6. Must pass pre-employment drug screening.
  7. Indian preference.

ESSENTIAL JOB DUTIES:

1. Ensures a smooth implementation and continuum of care via effective and frequent communication with care team members, patients, and identified health care designee.

2. Assist in ordering patient care referrals

3. Manage patient medication recall requests for assigned provider per medication refill protocols.

4. Maintain accurate and timely documentation in Nextgen EHR.

5. Conduct comprehensive assessment of patient’s physical, mental and psychosocial needs.

6. Works with care team members during huddle times to coordinate daily patient care schedule as well as same day appointments and double bookings as appropriate.

7. Manages assigned provider’s PAQ and task box

8. Manage and communicate to patients verbally or by telephone re: lab results, referrals, x-rays, medication issues per provider task.

9. Communicate with patients and assist with handling patient complaints

10. Assists with daily workflow to maintain smooth operation of daily activities, including reviewing appointments, ensuring patient flow, and following up on quality improvement measures.

11. Perform routine maintenance of the site crash cart

12. Actively participates in essential skills training

13. Perform or assist care team members in duties related to patient care ie; rooming patients, vital signs, HPI, point of care testing, wound care.

14. Assist with the maintenance and inventory of medical equipment, supplies, and educational materials, which may including (but not limited to), participation in monthly medication counts, medication inventory, crash cart medication and supply counts, ensuring all required in-clinic medication checks are accurate and complete.

15. Administer injections.

16. Render assistance to the clinic providers and perform specified duties

17. Maintain compliance with applicable laws consistent with clinic policies and procedures

18. Advocates for the patient/family to ensure the most compassionate and proper care is given.

19. Manage panel for chronic care needs as well as health maintenance.
